Making an Appointment

Step 1: Access

Verify network participation with your insurance carrier and verify your physical therapy coverage so that you are aware of any copays, deductibles and coinsurance for which you may be responsible, as well as prior authorization and referral requirements. Any applicable payments will be collected at the time of service.

Step 2:
Call Us

Call our friendly office assistant and schedule an appointment.
You will be asked the following questions:

  • What is the patient’s name and phone number?

  • Who is your insurance carrier?

  • What is your date of birth (DOB)?

  • Our staff will send you a link via text and/or email in order for new patient forms to be completed electronically (with no need to print documents to complete).

Step 3:
Prepare

Complete new patient paperwork electronically and gather the following information to bring to your first visit:

  • Your doctor’s referral order for Physical Therapy

  • Your health insurance cards and photo ID

  • Any imaging interpretation such as CT, MRI, X-ray, or post operative reports from your doctor

  • Method of payment

Please wear loose and casual clothing to permit assessment of the affected area.

Arrive 15 minutes before your appointment and check in at the front desk. You will be greeted by our front desk staff and asked for your insurance card, photo ID and physician order, if available.

Step 4:
Arrive

The first visit can take one to two hours.

Your physical therapist will interview you about your present problems and health history, conduct a thorough physical examination, and design a treatment plan specific to your condition.

Your insurance benefits will be verified and if you have a copay, it will be collected at this time. For your convenience, we accept cash, personal checks, MasterCard and Visa for payment (please note credit/debit cards will be assessed a 3.75% convenience fee).

A report will be forwarded to the referring physician after the initial visit followed by monthly progress reports. If deemed appropriate and authorized by insurance, we will administer treatment during the same visit after the initial evaluation is completed.

Cancellations

Please alert us 24 hours before your visit to cancel or reschedule your appointment to avoid a $40 no show/late cancelation fee.

*Injured Workers

Your physical therapy visits will not be covered if prior authorization is not obtained.

We must have a workers’ compensation claim number and doctor’s order in order for us to request authorization from your insurance carrier. Authorizations can sometimes take a week or two to obtain, so it is important that you work closely with your employer/HR department, case manager and claims adjuster in order to promptly obtain the care you need.

**Motor Vehicle Accident

For injuries related to an auto accident with an OPEN claim, please bring with you the following information:

  • Claim number, adjuster information and contact information

  • Responsible party’s insurance info, your personal auto insurance info

  • Attorney contact information, if retained

Most health insurance carriers will not pay benefits resulting from a MVA. If MedPay is available, we will first file claims to your auto carrier. If no MedPay is available, you will be asked to pay the cost of the visits upfront or set up a payment plan. A credit card must be kept on file for financial arrangements. If auto benefits are exhausted, we can bill your health insurance, but a “Benefit Exhaustion” letter from auto insurance must be presented to us as required by most insurance carriers.
