  • Please verify your insurance coverage so that you are aware of  co-pays and deductibles for which you may be responsible. We will bill your insurance carrier and attempt to get the claim paid. However, if your insurance carrier does not respond or denies your claim, you are responsible to contact your insurance carrier to resolve the problem/set up payment arrangements.
  • Our cancellation policy is as follows: After (2) two consecutive cancellations or "no shows" for scheduled appointments the patient may be discharged from the physical therapy program. The physician on record will be notified of any such noncompliance and discharge of patient.
  • Please be on time for your appointment. We wish to complete your entire treatment but at the same time we do not wish to delay therapy for other patients. For your convenience we accept Visa/MasterCard/Discover.

Direct Access or Referral
       In the state of TN you have the right to see one of our Physical Therapists without a prescription from a physician.  TN law calls it "Direct access".  The exception to this rule are BlueCare patients- they must obtain a written prescription or referral from a doctor.
      
       We can accept referrals from Medical Doctors, Chiropractors, Dentists, Nurse Practitioners, Physician Assistants and Podiatrists. 
The prescription/referral must include your name, diagnosis, date and has to be sign by referring professional.


                     We accept most insurances.

For scheduling an appointment we will ask you to follow these simple steps:

       Step 1
Call our friendly office assistant and schedule an appointment. You will be asked the following questions:
  • Who is your insurance carrier?
  • Who is your referring physician, if not a direct access patient
  • Is this a Bureau of Workers' Compensation Claim? If so, what is your claim number and date of injury?
  • What is your diagnosis? What part of your body is bothering you?

      Step 3
On your first visit please bring:
  • Your prescription for Physical Therapy
  • Your insurance card
  • Your completed paperwork
       Please wear loose and casual clothing to permit evaluation of the involved area.
        The first visit can take one hour. Your physical therapist will interview you about your present problems and health history, conduct a physical examination, and design a treatment plan specific to your condition.
       A report will be forwarded to the referring physician after the initial visit followed by monthly progress reports. If deemed  appropriate we will be able to administer treatment during the same visit after the initial evaluation.
